Benefits

  • Medical, dental and vision insurance plans currently provided by United Healthcare and Ameritas. Plans start at $37/month for the healthcare benefit premium, $10/month for the dental care premium, and $8/month for vision. There are options (at additional cost to the employee) for plans with additional benefits as outlined in the benefits guide attached. Employees are eligible for benefits on the first of the month following the date of employment.
  • A 401(k) Retirement plan in which employees may make pretax or after-tax salary deferrals up to the annual IRS limits. Employees are eligible to participate after 6 months of employment. Once eligible, CCAM will contribute a safe harbor contribution of 3% of your base salary. This contribution is 100% vested. The company may also make a discretionary contribution to eligible employees. In the past the company has made discretionary contributions of 7.5% of an employees’ base salary. This contribution is subject to a 4year graded vesting schedule. Please note that this information may change during your employment so please consult your 401(k) Plan document and summary plan description for more information.
  • Life insurance in the amount of 2x the employee’s annual base salary up to $300,000 benefit at no cost to the employee. Coverage begins on the first of the month following the date of employment. Our policy is subject to automatic reduction in benefit for individuals above the age of 65 (eligible for 65% of benefit), 70 (40% of benefit), and 75 (20% of benefit).
  • Long- and short-term disability coverage of 60% of your base salary at no cost to the employee. Coverage begins on the first of the month following the date of employment.
  • 8 paid holidays.
  • A Paid Time Off (PTO) Plan that will grant you 15 days PTO annually. Employees may carryover up to a total of 10 PTO days for use in future years.
  • CCAM provides all employees a $50/month allowance (or your actual monthly cell phone cost if less than $50) for your cell phone. You must submit this on an expense report for reimbursement each month.
